Care Navigation Manager

Care Navigation Manager Responsibilities :

  • Interact with current and prospective patients, engage with patients not connected with care, including in the initial patient consultation for our coaching program
  • Support providers and patients in our coaching program and value base care partnerships
  • Help expand our coaching program
  • Oversee report data from our external partnerships, identify gaps in care for patients, manage deliverables, and implement needed actions.
  • Performs other appropriate duties and participates in other special projects as assigned, including, but not limited to, quality improvement, community health education, facility/provider relations
  • Responsible for coordination and services for speciality programs with our external partnerships
  • Train and develop the coaching program
  • Completing and maintaining documentation in a timely and accurate manner and according to Federal, State and company guidelines

Care Navigation Manager Qualifications:

  • Masters Degree in psychology, social work, counseling, and or equivalent experience in the mental health field.
  • Proficient in excel
  • Collaborative problem solver
  • Strong engagement and assessment skills to work with a diverse range of patients
  • 1 year of experience with documenting and treatment planning within an EMR
  • Access to a computer, webcam, reliable internet connection and private, quiet space
  • Active licensure preferred but not required
  • Bilingual in Chinese or Spanish preferred but not required
  • Prior telehealth experience
  • Comfort with working on a remote team
  • Proactive, curious, & creative mindset: being able to identify deficiencies and swiftly executing on a solution
  • Strong interest in working in a fast paced, startup environment
